VNA is the largest primary care provider for people in need in the Dunham Fund service area, serving more than 30,000 low-income adults and children in Aurora alone. Approximately 10,000 of these individuals have no insurance of any kind. When uninsured people receive less medical care and less timely care, they have worse health outcomes.
This grant from the Dunham Fund allows VNA to implement its new technology, which equips nursing staff to track clients who go to the emergency room or are admitted to the hospitals in the area in real-time. New technology helps VNA to serve a higher number of clients in a more timely manner, visiting patients in their homes, and helping to reduce readmission rates.
The Dunham Fund is proud to continue partnering with VNA Health Care to ensure that people in our community receive proper care following a hospital stay.
